Work Item is triggered or sent twice

Hi WF experts,

I always get 2 notifications every time I reset release my PO.

This is my custom task - is there something wrong here?

This is what I do - ME29N then click the cancel release button

This is my inbox - as you could see the top notifications they always have 2 same dates and 2 same time stamps.

Please help!!!!

Why is this happening??
